How do you all prepare for when your sons get discharged from the hospital but you can tell they aren’t stable yet? I’ve listened to a ton of your podcasts and they are so enlightening- but I’d be interested to hear more of the technical details of transitioning a loved one out of or into the hospital and out of/into group homes for someone not yet familiar with navigating the patchwork of mental health services. 

What would we tell providers if we could?
